Student Health History

If your student has any significant health history please advise the school nurse. It is very important that we know of any conditions (diagnosed by physician) such as diabetes, asthma, allergies, seizures, or any other possible life-threatening conditions. If the child requires medication, over the counter or prescription there must be a medication permission form signed. If you need a copy please see the school nurse. Also depending on the condition an action plan or Individual health plan will be provided by the health care provider/school nurse and the parent to ensure your child’s safety. If you have any questions please contact the school nurse.

Medication at School

If your child requires medication at school, the parent must sign a medication permission form filled out for each medication in order for the nurse or select staff to administer it. The medication must be BOUGHT BY THE PARENT AND GIVEN TO THE NURSE (or office staff). You may not send medication with the child in their backpack. Doing so is dangerous and can cause harm to children. At the end of the year the parent must pick up any remaining medication in person or it will be discarded.

All medications must be in the original container and labeled with the child’s name and date of birth. If it is a prescription medication it must be in the original container with the original prescription label. No expired medication will be accepted / administered.

Immunizations

All students must meet immunization requirements per state regulation to attend school. Please bring a copy of the most up to date immunization record to the office /school nurse. All proof of immunizations needs to be on file no later than ___________.

Vision and Hearing

The school nurse will check vision and hearing for PK, KG, 1st and any students who are first time entrants who has not had a screening in the last year. If it is determined that your child might need glasses the nurse will send a referral. If you get a referral please have the eye doctor fill out paperwork and you must return it to the nurse. If the child needs glasses, another screening will be done with the glasses in school.

Clothing

Be sure your child knows how to undress for using the restroom. Have them practice belts, buckles or other tricky items before sending them to school with those items. IF your child is prone to wetting or soiling accidents please send and extra pair of clothes in their backpack.

Nurses office

Dillman Elementary has a nurse on campus daily, however there may be days that the nurse is out. If this is true there is trained staff that can help your child. We ask that if your child is sick that you keep them home until they are well or you have seen a doctor and you have a return note. If your student becomes ill at school they will be sent to the nurse’s office to be checked. Notes will be sent home for minor occurrences. If you need to be reached for any reason we will call you. Please be sure that we have current phone numbers on file for you and any emergency contacts listed on your child’s file.

Please make sure all health problems/concerns are listed every year or we will assume past concerns or allergies were resolved.

As a school nurse we do not diagnose children but treat them for acute concerns at that moment. If you have concerns please follow up with your child’s primary care provider.

Attendance

At the end of each six weeks we will draw one boy’s and girl’s name per grade level to win an attendance prize bag, and we will also reward all the other students who have perfect attendance with a smaller prize. Prizes may change pending availability.

2nd Six Weeks – Candy bracelets
3rd Six Weeks – Miniature Slinkys
4th Six Weeks – Stamp
5th Six Weeks – Sticky hands
6th Six Weeks – Chalk

Each class also has the opportunity to earn a popcorn party and be recognized over the intercom once they fill their attendance poster.

Perfect attendance for the year will be rewarded at the end of the year assembly with a medal and with the student’s name being entered in a drawing for a large prize such as a scooter.

Prompt and regular attendance, which is missing three or less days from the start of the school year, will receive a prompt and regular medal at the end of the year assembly. Students that have had perfect attendance throughout their Pre-kindergarten (if applicable), Kindergarten, first grade and second grade school years will receive a bicycle at the end of second grade.

The Texas Education Code mandates that a child must attend at least 90% of the days the class is offered to receive credit. If excessive days are missed the parents (or guardians) may be required to meet with school personnel to determine how work can be made up and to discuss the progress of the child.

Shining Star

We like to celebrate our students’ accomplishments and our Shining Star program helps us achieve this goal. Students earn shining star certificates throughout the month. Certificates are earned by making good choices, returning homework, being respectful, following directions, etc… At the end of the month, we have a drawing where we select two students from each grade level as our Shining Star students. They earn a shining star award, and they have their picture displayed on the MISD App.

Pick Up and Drop Off Procedures

One of our top priorities at Dillman Elementary is the safety of each and every child. To create the safest environment possible for our students we have devised procedures we would like for staff, students, and parents to follow at the end of the day.

If your child arrives after 8:05, you will need to bring them to the office. All doors will be locked at that time.

If you need to change the way your child is going home the office will need to be notified no later than 2:45. After 2:45 your child will need to follow the guidelines you selected at the beginning of the year.

Do not text or email your child’s teacher with changes to student dismissal. Teachers are busy with instruction during the day and may not check email or text messages. All messages during the day need to be directed to the office or you may send a written note to school with your child. We will not accept changes over the phone due to security reasons.

Once buses are loaded students will not be allowed off of the bus. Taking students off of the bus causes the bus route to each school to be delayed and creates a huge safety issue for students.

1st and 2nd grade PICK UP is on Avenue E behind the building by the gym. Please park, and your child will notify the staff member on duty when their ride has arrived.

PK & KINDER PICKUP – and any 1st and 2nd grade sibling of PK and Kinder students

Pick up in the circle drive is for parents of PK and KINDER students. Please stay in your vehicle. Your child will be escorted to your vehicle. Parents of PK and kinder students who also have 1st and/or 2nd graders need to notify the teacher that your 1st and/or 2nd grader will be picked up with the PK/Kinder sibling in the circle drive.

Pick up in the parking lot on the South East parking lot is for parents who wish to park and stand along the sidewalk. Please wait away from the kindergarten doors. We need to be able to see the adult that each child leaves with. Parents of PK and kinder students who also have 1st and/or 2nd graders need to notify the teacher that your 1st and/or 2nd grader will be picked up with the PK/Kinder sibling on the South East area near the kindergarten doors.

Enrichment program where students have the opportunity to learn about robotics, botany, chemistry, etc...

Roving Chef-PK, Kinder, 1st, 2nd Grade students

Students work with our cafeteria staff to learn more about good eating habits and prepare a snack.

Kids Quest- 1st & 2nd Grade students

After school tutoring program where teachers work with students to meet their educational goals.

Six Weeks Incentive-1st & 2nd Grade

Eligibility: attendance, grades, behavior

Activities may include movie, extra recess, edible crafts, game rotations, bounce houses, etc...

Buddy Bags - PPCD-2nd Grade

Each class has a set of back bags that are sent home with students to increase literacy resources.

Bags are checked out on Mondays and returned on Fridays.
